SUMMARY:

The light/dark rhythm is closely linked to the sleep regulation process (sleep/wake), which has demonstrated a bidirectional relationship with several diseases, including depression. The chronotype, behavioral manifestation of the internal circadian clock system, when evening, has been shown to be a risk factor for the development of psychiatric disorders. Sleep problems are potentially present in the clinical histories of depressive patients. This is a cross-sectional, case-control study, with the aim of investigating the relationship between chronotype and sleep in patients diagnosed with depression (MD) (n= 16) compared with healthy subjects (CTR) (n= 29). The study patients were residents of the city of Maceió, Alagoas, and assisted in the research settings: Psychiatry Ambulatory of the University Hospital Dr. Alberto Antunes (HU/UFAL), Psychosocial Care Center Dr Rostan Silvestre, Psychosocial Care Center Noraci Pedrosa. To assess the diagnosis of depression, the “Mini International Neuropsychiatric Interview” (M.I.N.I - brief structured diagnostic interview for the main psychiatric disorders) was used. For depressive symptoms, the Beck Depression Inventory (BDI) was used, while the Horne-Ostberg Test (HO) and the Munich Chronotype Questionnaire (MCTQ) were used to determine the chronotype of the sample. For sleep parameters, the Pittsburgh Sleep Quality Index (SPQI) and the Epworth Sleepiness Scale (ESP) were used. Results demonstrated that patients have worse body mass indexes when compared with CTR (0.01188). There was a significant association between MD and the presence of sleep disorders compared to CTR (0.0012). HO/MEQ scores were significantly lower in the MD group (p=0.018) indicating a trend toward eveningness similar to previously reported literature. Depressed patients take longer to get out of bed during the week (Wilcoxon= 0.023 and take longer to get out of bed also on days off (Wilcoxon = 0.01). Circadian disorders and poor sleep quality have already been considered precursors of disorders of mood in previous studies, as well as depression has been considered a risk factor for poor sleep. In the same way, nutritional problems and afternoon chronotype are factors with greater risk of existing in samples with depression. In clinical practice, it is essential to research the chronotype and sleep quality of patients with depression so that professional care guidelines are implemented, as well as care and preventive conduct ideally in the general population.
