SUMMARY:

Amyotrophic Lateral Sclerosis (ALS) is a rare and fatal disease, with a challenging diagnosis and no effective treatment. The identification of suitable disease-specific indicators could increase the life expectancy and quality of terminal ALS patients. Moreover, biomarkers can aid ALS’ much-needed drug discovery by providing useful, measurable, and accurate information about the disease’s progression and drug response. However, to date, and after more than a decade of studies, no true indicator has been identified. microRNAs (miRNAs) offer several features compatible with ideal biomarkers, these are circulating molecules that can be analyzed from liquid biopsies obtained by minimal or non-invasive procedures (e.g.: blood extraction). A systematic literature search was performed in PubMed, Embase, Web of Science, and Virtual Health Library databases from January 2000 to December 2020 to retrieve all original differential expression studies that evaluated, via RT-qPCR, liquid-biopsy-derived miRNAs in ALS versus healthy individuals. Twenty-five studies met eligibility criteria, providing data regarding 331 different miRNAs. From 75 miRNAs assessed in independent data set, we found that only the hsa-miR-206 was consistently reported as up-regulated, whereas the hsa-let-7f-5p, hsa-miR-26a-5p, hsa-miR-425-5p, hsa-miR-3665, hsa-miR-1825, has-miR-4530, and has-miR-4745-5p, as down-regulated. The contradictory reports for the other 50 miRNAs may be ascribed to variations in experimental design (e.g.: tissue of analysis, unstandardized criteria of diagnosis, etc.) and/or limitations of the approach. The enrichment pathway analysis of the seven miRNAs consistently reported as dysregulated showed an overrepresentation of signaling pathways linked to mechanisms underlying ALS pathogenesis including neurodegeneration. Thus, the accumulated findings assessed in this systematic review point to seven miRNAs to be explored as candidate ALS biomarkers in further well-designed screening studies.
