SUMMARY:

Introduction: Viral respiratory diseases and their complications are among the main reasons for seeking health services worldwide, especially in children. When associated with pre-existing clinical conditions, hemoglobinopathies, particularly anemia, were the most prevalent, revealing that they may be associated with complications in these patients.

Objective: To evaluate the profile of patients hospitalized with viral respiratory infection and to verify the association between clinical outcomes and pre-existing anemia.

Method: Observational and retrospective cohort study with analysis of medical records of 122 pediatric patients hospitalized in a public hospital between 2023 and 2024. Respiratory samples were collected via nasopharyngeal swab, tested for viruses such as Influenza, SARS-CoV-2, and others, including patients with detected viral respiratory infection on a respiratory viral panel and with a complete blood count collected at admission. Variables analyzed included demographic, gestational, and environmental exposure data. The observed outcomes were length of hospital stay, radiological changes, oxygen therapy, use of antibiotics, complications, and deaths.

Results: 54% of the participants were male, with an average age and weight of 27 months and 11 kilograms, respectively. Anemia was found in 33% of the patients. SARS-CoV-2 was the most frequently isolated virus (53.6%), followed by rhinovirus (25.9%). There was a trend towards a higher incidence of complications (RR: 1.82; 95% CI: 0.81 – 4.11) and need for ICU admission (RR: 1.84; 95% CI: 0.86 - 3.95) in patients with anemia. When stratified by disease severity, we observed a trend towards longer hospital stays in the moderate and severe anemia group (RR: 1.69; 95% CI: 0.78 - 3.65).

Conclusions: Patients with viral respiratory infection and anemia showed a tendency to develop more complications and require ICU admission. Those with moderate to severe anemia tended to have longer hospital stays.
