SUMMARY:

Introduction: Mental health care involves not only welcoming those who need it, but also the whole strategy that involves the functioning of services, including economic analysis that includes the cost of the resources necessary for its functioning. Thus, economic analyzes in health make it possible to understand the relationship between the use and cost of health services, enabling greater clarity in strategic planning and decision-making in health management.Objective: To identify the use and financially estimate the cost of mental health care for children and adolescents from the perspective of the public provider at the Child and Adolescent Psychosocial Care Center (CAPSi) in Maceió. Method: This is a study with a quantitative, cross-sectional and retrospective approach, based on Economic Evaluation in Health with partial analysis using primary data, delimited from the perspective of a public health service provider, within the corresponding three-month time horizon. to July, August and September 2019, the direct costs being identified considering the measurement by measures of central tendency according to the use of the users of the chosen service, using the micro-costing approach (bottom-up) to value the cost. The participants in this study constitute three different groups: companions of service users, CAPSi professionals and SMS professionals from Maceió; that converge to provide information distinct from a single research instrument: the Sociodemographic and Use and Service Costs Inventory (ISDUCS). Thus, for descriptive analysis, absolute and relative frequencies and measures of central tendency were used, being they mean and standard deviation, respectively. Inferential analyzes were performed using Pearson's Chi-Square Test (χ2) and Pearson's correlation, verifying the correlation between categorical variables. Also considering the significance of p value <0.05 and all tests were applied with 95% confidence. Furthermore, this study was approved by the Research Ethics Committee (CEP) of UFAL, according to the Consolidated Authorization Opinion under number 3,566,665. Results: Individual consultations and therapeutic workshops totaled 68.1% of all procedures offered to the 726 patients who used this service. These users were mostly adolescents (60.9%), male (63.4%), frequent for more than 1 year (78.8%), mainly due to disorders related to hyperactivity / attention deficit (29, 1%) and autism (23.8%), using psychopharmaceuticals (93.4%) with greater use of antipsychotics (63.1%). They were accompanied by women (90.1%), generally the mothers (66.7 %), without employment (75.2%), low education (64.5%), some also with mental disorders (28 , 4%) and using psychotropic drugs (21.3%). These families were from the V Health District of Maceió (33.5%), were surviving on up to 1 minimum wage (58.9%) usually with access to the BPC / LOAS benefit (31.1%). The payroll represented a large impact (79.9%) in the total cost of operating CAPSi. Considering the total amount (R $ 701,776.34), it was possible to estimate the cost of each service performed (R$ 552.57), the cost of mental health care per user (R$ 966.63) and the cost of assistance in mental health per user in the therapeutic workshop (R$ 2,210.28). Conclusion: the knowledge of the direct cost of mental health assistance favors the possibility of rethinking the direction of expenses and allocating resources in a more applied way regarding the cost-benefit of various interventions.
