SUMMARY:

The rupture of the integumentary tissue produces biopsychosocial impacts, especially in conditions of delay in its repair process. The chronification of a wound is multifactorial and includes damage related to infections, which are responsible for the destruction of healing components. Among the main infectious agents in wounds, the Staphylococcus aureus species stands out, a microorganism recognized for its antimicrobial resistance. Strains with a profile of resistance to stable penicillins, such as oxacillin-resistant S. aureus, deserve to be highlighted for their ability to present combined resistance to different classes of antimicrobials and transfer resistance genes to other microorganisms. In this sense, the present study aimed to characterize the epidemiological profile of individuals with chronic lesions and to track the presence of Staphylococcus aures and its susceptibility to oxacillin. This is a cross-sectional, quantitative, descriptive study carried out from the collection of biological material from individuals with chronic wounds. Fifty-three adult individuals of both sexes were included in outpatient followup for treatment of one or more chronic wounds. The collection of sociodemographic data and clinical aspects took place through interviews, consultation of medical records and evaluation of the lesion. The isolated microorganisms were submitted to biochemical tests and culture in selective-differential medium for presumptive identification. The determination of the Minimum Inhibitory Concentration was carried out using the broth microdilution technique. The statistical analysis of the results took place in a descriptive manner. Among the 53 participants, there was a predominance of female individuals (58.49%), self-declared brown (47.17%), with a mean age of 59.6 years, low level of education (64.15%), with social security/ social (84.91%) and residents of Maceió (69.81%). In addition, most participants (90.57%) reported comorbidity, with the most frequent being Systemic Arterial Hypertension (77.36%) and diabetes mellitus (47.17%), a small portion of the sample reported smoking (7.55% ), routine use of alcohol (3.77%) and hospitalization in the last 12 months (20.75%). Regarding the clinical characteristics of the lesion, the most expressive group of participants had 10 years or more of evolution of the lesion (37.74%), single lesion (56.60%), classified as venous ulcer (64.15%), whose main location was in the lower limbs (96.23%) and with a size smaller than 10cm² (33.96%) or between 10 and 50cm² (32.08%). In addition, the presence of unfavorable signs in the evaluation of the perilesional skin (64.15%) and edge (60.38%) prevailed, as well as the presence of signs of infection (56.60%); while the presence of favorable signs, such as the presence of only viable tissue in the wound bed (62.26%), little (41.51%) or moderate (43.40%) volume of exudate and serous type (84, 91%), were also recurrent. The use of systemic antimicrobials was reported in a small portion (3.77%) of the participants, although all (100%) used topical antimicrobials. The presumptive identification confirmed the presence of eight strains of S. aureus, from 7 lesions/participants. Most of the isolates (75%) were sensitive to the lowest concentration of oxacillin evaluated (3.90ug/ml), although two isolates (25%) were only sensitive to higher concentrations (62.5ug/ml and 500ug/ml ). Thus, this investigation made it possible to characterize the sociodemographic and clinical profile of people with chronic wounds in outpatient care, whose presence of S. aureus was confirmed, including strains with resistance to oxacillin.
