SUMMARY:

Inflammatory bowel diseases (IBD), composed of Crohn's Disease (CD) and Ulcerative Colitis (UC) have altered immune response and oxidative stress, which are possible etiologies. Given the limitation and side effects related to traditional pharmacological therapy, the investigation of complementary antioxidant therapies has been the subject of several studies. However, it has low bioavailability and piperine, present in black pepper, appears as a promising potentiating agent in this perspective. In this sense, the study proposed to evaluate the action of oral supplementation with Curcuma longa extract and piperine on inflammatory markers and oxidative stress in patients with IBD.

This is a randomized, double-blind, placebo-controlled clinical study developed in the coloproctology sector of the University Hospital of the city of Maceió-AL, where 40 patients with IBD (out of 60 in total), according to inclusion, were randomly assigned to Curcuma longa extract three, Curcuma longa + piperine or placebo groups, for months. The dosage of Curcuma longa extract and piperine was 1g.d and 10mg.d, respectively. Personal, socioeconomic, clinical, anthropometric, dietary and biochemical data are collected. Laboratory analyzes consisted of evaluating the inflammatory profile (MPO, IL-1β, IL-6, IL-17, IL-22, IL-10, IFN-γ, TNF-α and NFκB), oxidative (SOD, CAT, GSH , Gpx, H2O2, MDA), in addition to routine clinical serum measurements (CRP, blood count) and fecal calprotectin. The project was approved by the Ethics and Research Committee of UFAL, under process nº 7829516.5.0000.5013. A partial characterization of the patients included in the study, assisted as outpatients, was performed. Among the 40 patients (UC: 28 and DC: 12) recruited so far, 67.5% are female and 32.5% are male, with a mean age of 45.55 ± 14.14 years. Most have a monthly income of more than one minimum wage (62.5%, n=25) and have had more than 4 years of education (65%, n=26). Regarding the duration of IBD and conventional drug therapy, 62.5% (n=25) have been diagnosed for less than 10 years and 55% (n=22) are using biological therapy. As for life habits, no one consumes alcohol, 22.5% (n=9) are smokers or ex-smokers, 45% (n=27) are sedentary and 57.5% (n=23) have problems with sleep quality. Abdominal pain, diarrhea and rectal bleeding, which are the most common gastrointestinal symptoms in IBD, were reported in 45% (n=18), 40% (n=16) and 25% (n=10), respectively. Fecal calprotectin (62.5%, n=25) and C-reactive protein (68.4%, n=13) levels were elevated in most patients, but with adequate hemoglobin concentrations (80%, n=18). With regard to nutritional assessment, although most patients had BMI values within the normal range (47.5%, n=19), the percentage of body fat was quite high (92.5%, n=37), indicating the importance of more sensitive tools to achieve a more assertive diagnosis and intervention. Therefore, the partial characterization of patients with IBD makes it possible to know the profile of those who are being assisted in the state and the factors that can influence the clinical course of IBD, in addition to providing information that will be considered in the final outcomes of the clinical study
